Objective

The major aim of the proposed interdisciplinary solution entails timeously identifying patients who can be prone to readmissions and using various strategies to reduce that risk. Some of the strategies may include teaching the patients and their family members regarding the importance of adhering to the medication regimen, following the prescribed lifestyle modifications, and ensuring that the patients get the necessary support during discharge to help them better cope with symptoms upon discharge. Therefore, the aim of the approach is to lower the rates of readmission by at least fifty percent.

Questions and Predictions

  1. What are some of the approaches which the interdisciplinary team will use to explore the patient and their family members’ learning and education? As part of the plan, the interdisciplinary team involved in the solution implementation will carry out a comprehensive assessment to help determine the patient’s learning and education needs as appropriate. The assessment will also be used to determine the patient’s learning styles and readiness to learn. In addition, the team will also be tasked with determining the patient’s literacy levels so that effective teaching strategies can be used at different points of care to help equip them with knowledge on the need to reduce the readmission rates.
  2. What strategies will the interdisciplinary team use to collaborate during the implementation of the proposed solution of educating the patients and their family members, especially during discharge? The interdisciplinary team will consult each other at various points of care and record patient details such as health beliefs, knowledge, and understanding to deliver the best possible education.
  3. How will the interdisciplinary team determine if the implemented intervention has led to the expected results of lowering the rates of hospital readmissions? As part of the plan, the team will use hospital data sources to help determine the reduction in the rates of readmission. It is important to note that the data on readmission before the use of the proposed intervention will be acquired and compared to the rates of readmission after the intervention. Such a comparison will help determine the impact of the intervention.

Change Theories and Leadership Strategies

Change theories offer a framework upon which intervention strategies can be implemented. The PDSA model has been chosen for this project. The PDSA is a change theory that has four major phases, as has been highlighted (Krudsen et al., 2019). The first phase is a plan which involves formulating a plan which can be followed. The second phase is do, which entails the commencement of the implementation phase. The third stage is studying where data gathered in the first two phases are considered to reveal if an improvement has been observed and the observed trends. The last phase is Act. In the last phase, if the implemented solution resulted in success, then it is standardized and implemented as a new quality improvement initiative for better results.

It is important that every member of the interprofessional team participates and carry out their duties as appropriate for the implementation to succeed. This model gives the team a chance to restart the process in case the data obtained from monitoring is not desirable (Connelly, 2021). The leadership strategy that will be used in the project is the transformational leadership approach. This leadership style or strategy has been proven to support interdisciplinary collaboration. The proposed solution is an interdisciplinary solution; therefore, this leadership strategy is the best fit.

The implementation of the interdisciplinary solution requires an appropriate collaboration between the interdisciplinary team. Such collaboration would improve the chances of achieving the plan’s objectives. Therefore, it is important to use effective team collaboration strategies. One of the strategies includes open and effective communication, which has been shown to improve coordination between teams (Klein, 2021). Through this approach, the team members will be able to positively contribute through ideas, thoughts, and feedback.
